I’m assuming they call those “jeweled skulls”, and it is a Japanese thing (as opposed to “sugar skulls”, which is more of a Mexican thing). Check out Jeff Goguey’s work – he’s done a couple of those.

It’s also, depending on your artist and/or aftercare, not abnormal for a little color to be missing. Usually your artist will be happy to do touch-ups on those areas for free.

I dunno. It’s not a traditional tattoo style, but the picture is really too dark and too far away to get a good idea of the linework. Has the artist finished with it, or was he/she planning on shading/coloring/other?

Can you shoot a closer picture of the girl?

There are several styles of filler which could be done in light grey and provide a background. Lacework, organic arrangements, stylized geometry – any one of which could provide more coverage without detracting from the lettering itself, as long as it’s light enough (like shade).
