Ah, okay. I can believe both places are very painful indeed.
My biggest concerns today, since I seem to be heading for 4 full sleeves, (should you choose to call the leg pieces ‘sleeves’) are basically those 4 (8) places.. elbows, knees and the insides of both. It’s the second thing after money to stop me wanting sleeves done ๐

Personally I think if you’re going with flames, make them flame colored… not pink, gray and cyan. Appart from that, I don’t like the tattoo one bit. The flames are… ugly. The top left one looks more like a human running than fire.

Since a Koi is a relatively common tattoo, there should be examples of Koi in their portfolio. If there aren’t any, IMO you should find a different artist.

My opinion might be skewed by my general dislike of tribal, but I agree that this could look really wicked if it were done in some nice fading colors and shading.

It looks like 6 different people drew it on the foot with a sharpie, and then someone inked into the sharpie lines.
