No, the moles won’t be a problem at all.

The only reason they would stick out is if you were getting really lite shading done directly on the moles: but even then, it wouldn’t look bad at all.

If you don’t have any basis for an idea…I doubt any artist would have any ideas.

What kind of theme are you thinking?
What is the current piece you have? Maybe you could extend on that theme?
