If you are changing your design…that is probably not going to happen.
The only way that would work is if your outline work wasn’t very complex and doesn’t cover much…or your tattoos are VERY dark that will cover it.

That font especially is a bold statement on its own.
If you were to add to it; it would lose that vibrant type-face that makes it so robust (and cool) in my opinion.

For the first piece…
I suggest an anatomical heart instead.
And I’d change the font as well…those thin lines are not only slightly hard to read…they will bleed out over time and become a lot smudgier.
And personally, I like the piece…but I’d suggest letting your artist put their own twist on it.
