Some areas hurt a lot more than others. I find that the feeling can range anywhere from something like a cell phone vibrating on you to being carved into with a white hot butter knife. Or worse.

Like Sam said, that will depend on your definition of old. I think that when you are very very old the skin gets very thin and that makes it harder to tattoo. But I have seen pics of some very elderly people, like in their late 80s or 90s, smiling as they get their first tattoo.
