the design is beautiful . just be 100% sure before you get it done because it will seem like a big change done very suddenly .

i wouldn’t cover it up it can easily be worked into a sleeve

it is indeed possible by a good artist , the main problem i would worry about is that your tattoo idea sounds like something a woman would have more than a guy .
