Never looked decent in the first place. as Dan said get it covered

Just find the best tattooist you can to do it. Then come back here and post a link to the tattooists work and let the people here that know what to look for approve of your choice.
Nobody here is going to design your tattoo for you

Yes the tattooist that is going to be doing the tattoo. This is their job and most true tattoo”artists” would prefer that they drew it.

Sounds like you’re on the right track with this but maybe do two separate tattoos.

The most important part of getting your new tattoo is the tattooist that you choose

Laser first then cover it up with a proper tattoo

It looks straighter in this pic

Less is better when it comes to putting stuff on it, It’s a wound and needs to breath a bit.

From this picture it sure does look crooked. maybe it is the way your standing or head is turned a bit. Take more pics from all different angles and see if it changes.
Should be fairly easy to cover up with a proper tattoo.
How did you pick your tattooist?

No you should get it lasered and covered up. It is done poorly it is unreadable it is off center and unevenly spaced.
